在近代日本坊间流传着这样一个传说，名叫岛礼子的美丽少女被美国大兵轮奸，在此之后她精神失常，卧轨而死。岛礼子一缕幽魂不散，化作只有上半身的恐怖恶灵。它极端憎恶红色，每晚出没在横跨铁路的天桥上，并在路过之人背后发出“嗒卡嗒卡”的声音。一旦路人回头，它便将对方拦腰斩断。即使侥幸逃过，在接下来的72小时内也必然遭到“嗒卡嗒卡”的猎杀。
高中少女大桥可奈（大岛优子 饰）的好友绫花夜归遭遇“嗒卡嗒卡”，遇害身亡。克奈在祭拜好友时，同样见到这个恐怖恶灵，不过侥幸逃脱。然而死亡的阴影仍然笼罩在她的头上，在女大学生平山理绘（山崎真实 饰）的帮助下，她们一同出发寻找求生的办法……
