来自南美的马克斯·伯雷表面上是个海上贸易的商人，暗中却从事军火、毒品、绑架甚至强迫卖淫等罪恶勾当。某国接连派出四名特工前往他所落脚的伊朗首都德黑兰，却无一例外，皆遭杀害。为避免公开行动引起国际纠纷，该国聘请神秘的杀手“骷髅13”（高仓健 饰）前去刺杀马克斯。骷髅13本名Duke·东乡，是一名混血儿。经过慎重考虑，东乡接下这个任务，并和该国女特工凯瑟琳假扮夫妻潜入伊朗。他的行踪不久便为马克斯的团伙所得知，对方相继派出瓦尔特、西蒙、道格拉斯等高手前来阻挠。高手对决接连展开…… 本片根据さいとう・たかをが的漫画改编。
